The Family Care Provider Network

Milwaukee County Department on Aging's Care Management Organization (MCDA/CMO) contracts with the Wisconsin Department of Health and Family Services to provide services for Family Care members. 

The MCDA/CMO has a network of providers that are under contract or employed by the CMO to provide services in the Family Care Benefit Package. 

As a contracted provider, your name is added to the service provider list.

CMO Members and their care management team choose service providers from this list.

The CMO is required to monitor the Provider Network to ensure we have enough service capacity to meet the needs of current and anticipated numbers of members. If there are too many service providers in the Network, our administrative costs increase and our ability to monitor quality decreases. 


The CMO may close the Provider Network in certain services areas, when capacity and demand for services are equal.
