Case Management

All City of Milwaukee children reported with capillary (fingerstick) or venous blood lead levels equal to or greater than 10 µg/dL receive CLPPP follow-up services. Children with a venous blood lead level of 15 µg/dL or higher and children age 0-36 months with a venous blood lead level 10-14 µg/dL or a capillary blood lead level 10-19 µg/dL receive nursing services from either a Public Health Nurse (PHN) or a Health Services Assistant (HSA) according to the City of Milwaukee Health Department Chidlhood Lead Poisoning Prevention Program (CLPPP) Intervention Schedule (see below).

 

CLPPP Nursing Service goals are:

  1. Ensure that the medical and health needs of lead poisoned children and their families are met.
  2. Ensure a lead poisoned child's growth and development are assessed and monitored.
  3. Ensure appropriate referrals are made.
  4. Educate parents and caregivers, empowering them to appropriately manage their child's health, resulting in prevention of further lead exposure.


Children with a venous blood lead level of 15 µg/dL or higher require multi-service coordination by a PHN assigned to the CLPPP. The PHN collaborates closely with an Environmental Lead Risk Assessor who inspects the child's environment for lead hazards. The PHNs are supported by a by a PHN Lead Coordinator and a PHN Supervisor. Direct services are delivered in the client's home until the nursing service goals are met, if possible.

Children age 0-36 months with a venous blood lead level 10-14 µg/dL or a capillary blood lead level 10-19 µg/dL receive an Early Intervention home visit by an HSA, which includes education, a walk-through environmental assessment, and wet washing and/or HEPA vacuuming to remove immediate lead hazards.

Families of children who do not meet the criteria for a new case or an early intervention case are provided educational materials.
