Core Team Public Health Nursing

Public Health Nurses have been caring for our community for over 100 years. 

What is Core Team Public Health Nursing?
Core Team Nursing promotes the health of individuals and families through home visits, clinic visits, and telephone consultations.  They act as both a community resource and as an advocate for families and individuals.  Core Team Nursing also offers health assessments, counseling and Safe sleep counseling through our Cribs 4 Kids Program in our Family Health Clinics.  Clients are seen in Family Health Clinic by appointment only.

What is the goal of Core Team Public Health Nursing?
The primary goal of Core TeamPublic Health Team Nursing is to reduce infant mortality.  We plan to do this by:

  • Advocating for our clients to maintain a healthy lifestyle
  • Educating on the importance of a safe sleep environment
  • Improving infant and child care through education and assessment
  • Improving prenatal care and education
  • Preventing communicable diseases of infants & children via immunizations

Who is eligible for our services?
Any City of Milwaukee resident is eligible to receive services from us.  Our primary focus for home visits is to pregnant women, high-risk infants, and children needing additional health or developmental assessment.
