Milwaukee County's Family Care, Care Management Organization respects the dignity and personal autonomy of each Member by honoring choice and promoting the Member's continued participation in the life of their community, by providing a continuum of quality cost-effective long-term care to its Members, and by supporting the families and caregivers of its Members.
About Family Care
Family Care is a long-term care program now available in nine Wisconsin counties. As a comprehensive and flexible long-term care service system, Family Care strives to foster people's independence and quality of life, while recognizing the need for interdependence and support.
In Milwaukee County, Family Care offers managed long term care services to adults aged 60 and older.

When you enroll in Family Care, you will work with a Care Management Team. Your Team works with you to list your personal goals and the resources you need to create a plan for your primary health care. A care management team includes:

Eligibility for Family Care
Family Care is an entitlement. That means eligible older adults are entitled to its services. In Milwaukee County, participants must be at least 60 years of age, have long-term care needs and qualify for Medicaid (T-19).
Cost share
Some Family Care members may have to pay for part of the services they receive. This payment is known as cost share. Cost share is based on the member's income and the cost of the member's care.
Room and board (food and rent) is not a covered benefit under Family Care. If you live in an assisted living facility, you must pay the cost of room and board. If you move to an assisted living facility, you are responsible to pay for the room and board costs of the facility. You must pay for your meals and the cost of your room or apartment. Your team will help you determine if your choice of living arrangement is affordable for you.
How to Enroll in Family Care
Enrollment in Family Care starts with a phone call to the Aging Resource Center of Milwaukee County at (414) 289-6874. Resource Center staff will arrange to meet with you to explain more about Family Care and to see if you are eligible for the program.
