HEALTH INSURANCE

In 2009, the City offers two health plans to eligible employees:

    1. UnitedHealthCare (UHC) Choice Plan
      After Open Enrollment, call the number on your member ID card.  UHC will offer two benefit structures based on labor contracts in 2009.
    2. Basic Plan (administered by Anthem Blue Cross/Blue Shield CMS)
      • Basic Plan Tier 1:  This option is available to Local 494 (Electrical) and MBCTC employees only in 2009, and only until a new contract is settled.

City employees must use the online Employee Self Service program to enroll in 2009 benefits.   To login to Self Service, click here.  For Open Enrollment Self-Service Instructions, click here (pdf).  

Only employees of HACM, WCD and MEDC should use the following Health Enrollment forms in one of the following formats:  Microsoft Word  /  Adobe PDF


BASIC PLAN (Anthem Blue Cross Blue Shield):

  • How to find a doctor:
    1. Go to http://www.anthem.com and click on Find A Doctor.
    2. Select Wisconsin then click on the Next button.
    3. Under "Select a Plan" click on the drop-down menu, and select Blue Perferred Plus (POS).
    4. Select your "Provider Type" from the drop-down menu.
    5. Select a "Speciality" from the drop-down menu, and click the Next button.
    6. You will then be sent to the Provider Finder page where you can find a Doctor according to your zip code or other criteria.

If you have questions regarding your benefits, unpaid bills, or problems with service, please call your health or dental plan. DO NOT call Employee Benefits until you have contacted your health or dental plan and are unable to arrive at a resolution. Employee Benefits will attempt to assist you to resolve your problem, but in no case will Employee Benefits attempt to change, question or provide a medical opinion. Remember to document all your conversations with dates, times and names. We will ask you for this information when you call our office.
